Signature Home Health Care brings many of the services offered in a hospital or outpatient facility directly to the comfort and convenience of your home. Working under the supervision of your personal physician, our staff of highly-skilled medical professionals can address even the most complex of needs:
Certified Family Nurse Practitioner: Providing supervision and coordination of care.
Registered Nurses: RNs provide direct patient care including assessments of your overall health and condition, making sure wounds are healing well, providing additional instruction about medication, providing additional instruction about therapeutic diets, and much, much more. RNs can provide home infusion and needed injections. RNs can also perfrom all activities of LVNs.
Licensed Vocational Nurses: LVNs are nurses who excel at your day-to-day care needs while you are recovering from injury or illness at home. Your LVN may keep track of your vital signs, help you take your medications, set up your pill box, collect specimens for lab tests, and more.
Physical Therapists: Our physical therapists will travel to your home and ensure that, after your illness or injury, you recover the maximum physical ability possible. Signature PTs will design safe, effective exercise programs that are specific to your current challenges and that you can perform at home.
Occupational Therapists: Our OTs are specialists at maximizing your ability, even when strength or mobility limitations exist. They may devise therapeutic exercise plans for complex areas such as hand rehabilitation, but our OTs are also exceptional at devising strategies for using equipment and home modifications to make the home environment work for our patients.
Speech Therapists: People often think of speech therapies ability to improve the ability to speak after a stroke and other injuries. In addition, Signature speech therapists can help with feeding, chewing, and swallowing difficulties. STs also excel at cognitive exercises that help restore memory and the connections between the mind and the capacity for speech.
Medical Social Workers: MSWs help identify other resources in the community and coordinate their implementation. They also can facilitate the handling of other legal-health-financial issues such as organizing the medical power of attorney documentation.
Home Health Aides: HHAs visit the homes of our patients to assist with health-related activities of daily living while our patients may be unable to do these things for themselves. This may include handling our patient's laundry, changing the bed linens, assistance bathing, etc.

Signature Home Health Care delivers a premium quality service often at very little to no cost to you. For qualified beneficiaries, Medicare pays 100% of allowable charges from Signature Home Health Care. We accept Medicare, Medi-Cal, and private insurance. Our services are covered in full in most cases.
